If your child is required to take/store medication at school, please fill out the following form.
2025-2026 Medication Management Plan
Medication will be kept safe in the school office and must be in the original packaging or prescription container, with the child's name on it.
If your child has epilepsy, asthma, or an anaphylaxis allergy (Epi-Pen) please complete the following form.
2025-2026 Medical Emergency Plan
If administering medication, such as Benedryl, is part of your Emergency Plan, please fill out the above Medication Management Plan form aswell and send the allergy medication in to the school office.
Please put your child's name on the medication.
